We authorize access to Personal Information to authorized personnel only and in accordance with the HIPAA Notice of Privacy Practices and Informed Consent terms also available on this site for your review. While we strive to protect your Personal Information, we recommend you take every precaution in protecting your Personal Information when you are on the Internet.
